Here’s our Smile Pop Invisalign Consent Form that every patient must read, understand, and sign before starting treatment with us. 

My Invisalign App

It’s not a bad idea to download the My Invisalign App and use it during your treatment, however, DO NOT ACTIVATE it until you have had your Invisalign Start appointment at Smile Pop and receive your unique patient ID that is listed on every bag of your aligners.
